Established in 1992, Primary Care & Hope Clinic has existed as a comprehensive health care safety net program for the medically needy and uninsured. The mission of the clinic is to improve the health and well-being of the communities they serve by providing excellent and compassionate care, accessible to all. Primary Care & Hope Clinic provides a medical home for the uninsured and underserved that consists of the sustained and integrated delivery of preventive, acute, chronic and behavioral health services, regardless of the patient’s ability to pay. The agency’s goals/outcomes for the year include: Primary Care & Hope Clinic will provide a medical home for the uninsured, underinsured and/or underserved, as indicated by a patient­to­visit ratio of at least 2.8. This ratio indicates that patients are coming in for multiple visits, returning for follow-up care, lab work, and refilling their prescriptions. This is an indicator that the patient is working with their health care team to improve their health, rather than being seen only for urgent issues. 20% of behavioral health patients will show at least a 5-point improvement on the PHQ-9 depression scale.
