Charis Health Center

Established in Mt. Juliet, Tennessee in 2008, Charis Health Center has a mission to provide effective and affordable health care to adults and children who are vulnerable and medically uninsured. The funds requested in the application will support the salaries and payroll benefits for nurse practitioners. 425 out of 500 (90%) uninsured patients will be compliant to treatment and medication plans. 160 out of 200 (80%) of women will receive breast health education during their well-woman exam. 50 out 200 (25%) of patients needing a mental health referral, will receive treatment within the clinic at time of visit or within a week after.
