Charis Health Center received funds to support improved access and health outcomes for the medically uninsured. The agency’s outcomes include: 75 out of 100 (75%) of new patients identified with hypertension will decrease blood pressure to elevated or normal range; 75 out of 100 (75%) of patients identified with diabetes will have controlled A1C and patients with poor A1C will have controlled A1C levels within three months; and 400 out of 500 (80%) of female patients age 40+ will receive and complete well-woman exam with breast and cervical cancer screening within 12 months of last screening or new patient visit.
