Founded in 2001, the mission of Faith Family Medical Center (FFMC) is to follow the commitment of people of faith to provide hope and medical care for the working uninsured and other underserved by meeting their physical, emotional and spiritual needs. FFMC has served more than 35,000 Middle Tennessee patients through medical care, individual/family counseling, and wellness programs. By integrating comprehensive wellness and chronic disease management with medical care, FFMC promotes active, healthy living through physical fitness, nutrition education, and behavior modification. The agency’s goals/outcomes for the year include: 50% (70 of 140) of diabetic participants will achieve and/or maintain an A1C value of less than 7%. 50% (120 of 240) of participants diagnosed with diabetes, hypertension, hyperlipidemia, or obesity will achieve and/or maintain an average blood pressure of less than 140/90.
