Founded in 1943, Family & Children’s Service (FCS) serves all people in crisis and transition by meeting them where they are, understanding their needs, and providing them with the resources they need. The mission of Family & Children’s Service is to connect individuals and families to hope, to healing, and to one another. FCS often “fills in the gaps” in social services, creating a safety net to ensure that all children and families can be safe, healthy, and connected. The funds requested in the application will support salaries and benefits for Healthcare Access Specialist staff positions in FCS’s HealthCare Access programs. A minimum of 892 of 1,115 (80%) of case management clients will secure affordable insurance, successfully access community-based affordable healthcare, and/or find a healthcare ‘home’, as indicated by case file records. A minimum of 210 of 300 (70%) of Health Assist Information & Referral clients will indicate at the close of the call that they have successfully identified a resource(s) to: (1) secure affordable insurance, (2) successfully access community-based free/affordable healthcare, and/or (3) find a healthcare ‘home’, and have the information and means needed to access that resource(s).
