Founded in 1943, Family & Children’s Service (FCS) serves all people in crisis and transition by meeting them where they are, understanding their needs, and providing them with the resources they need. The mission of Family & Children’s Service is to connect individuals and families to hope, to healing, and to one another. FCS often ‘fills in the gaps’ in social services, creating a safety net to ensure that all children and families can be safe, healthy, and connected. Each year we provide direct services to more than 6,000 clients, and phone-based crisis and referral services to another 30,000 Tennesseans. The funds requested in the application will support staff positions and related expenses to assist clients in enrolling in health insurance and/or obtaining other health services to meet their needs. A minimum of 1,086 of 1,358 (80%) of Health Assist Tennessee (HAT) case management clients will secure affordable insurance, successfully access community-based affordable healthcare, and/or have found a healthcare ‘home’, as indicated by case file records.
