HOPE Family Health is a faith-based nonprofit health center located in rural northern Tennessee, with a service area encompassing Macon, Sumner, and several surrounding counties. Founded in 2005, HOPE has over 1,000 patient encounters per month. The agency’s goals/outcomes for the year include: out of 556 adult diabetic patients, 70%, or 389, will have Hemoglobin A1C’s below 9%. Of the 558 new adult patients, 70%, or 391, will be screened for substance abuse and, if interested, referred for treatment. Of the 795 patients projected to use HOPE’s behavioral health services, 318, or 40%, will receive counseling services with a licensed therapist (up from 33% in early 2016).
