Partners for Healing originated in 2001 when community leaders realized that few if any healthcare options existed in the area for working uninsured. Using volunteers and local donations of time, talent, and funds, the organization obtained an earned reputation of providing compassionate care and nurturing to the working uninsured in their area. The funds requested in the application will support expansion of the chronic disease self-management program in four areas: education, nutrition, exercise, and spiritual/mental well-being. 35% (102 out of 290) of patients will participate in the program. 80% (82 out of 102) of participants will rate program satisfaction as good or excellent. 50% (51 out of 102) will have a HgbA1c of seven or less.
