Established in 1992, Primary Care & Hope Clinic is as a comprehensive health care safety net program for the medically needy and uninsured, providing physical examination, provision for laboratory work, pharmacy, X-Ray, behavioral health care, substance abuse treatment, specialist referral and care coordination. The agency’s goals/outcomes for the year include: providing a medical home for the uninsured, underinsured and/or underserved and improving the depression of 20% of its behavioral health patients by at least 5-points on the PHQ-9 depression scale.
