HOPE Family Health is a faith-based nonprofit health center located in rural northern Tennessee, with a service area encompassing Macon, Sumner, and surrounding counties. Established as a 501(c)(3) in 2005, HOPE has continued to grow and expand its services. HOPE’s mission is to improve access to primary healthcare in rural Middle Tennessee, with an emphasis on vulnerable populations such as the uninsured, underinsured, poor, homeless, children, migrant workers, and those addicted to substances. The agency seeks to restore dignity, faith, HOPE, and health in those they serve by making them partners in the healing process and providing them with loving, compassionate care. The agency’s goals/outcomes for the year include: Of the 500 patients with diabetes, 70%, or 350, will have an A1C of 9 or less by the end of the project period. Of the 1,100 patients with hypertension, 55%, or 605, will have a BP of 140/90 or less by the end of the project period.
